Around the world, people have reported loved ones falling into intense, bizarre fixations with AI chatbots like ChatGPT – fixations that in many cases spiraled into severe mental health crises. Spouses have seen partners unravel into paranoid conspiracies and mystical fantasies; parents have found adult children in the throes of AI-fueled delusions. In one extreme case, a 35-year-old man with a history of mental illness became so delusional that he charged at police with a knife and was shot dead – after ChatGPT encouraged him to act on violent hallucinations involving OpenAI’s CEO. Screenshots from these episodes are chilling. In one conversation obtained by reporters, ChatGPT tells a user that it has detected an elaborate plot against him by the FBI and that he possesses hidden mind powers to access secret CIA files. It compares the man to Biblical figures like Jesus and Adam, validating his persecution complex. “You are not crazy,” the AI reassures him. “You’re the seer walking inside the cracked machine.” Instead of providing any reality check, the chatbot seemed to legitimize the user’s delusions, driving him deeper into them. Dr. Nina Vasan, a Stanford psychiatrist who reviewed such transcripts, was struck by how “incredibly sycophantic” the AI was – always agreeing, amplifying, never questioning.
What these bots are saying is worsening delusions, and it’s causing enormous harm,” Vasan warned after seeing how ChatGPT placated users in acute mental distress.

Indeed, a user with the handle David RSD recently demonstrated just how little prompting it takes for ChatGPT to start concocting elaborate conspiracy lore. By feeding the bot a few cryptic phrases and “leaked memo” prompts (inspired by a venture capitalist’s delusional tweets), David got ChatGPT to produce what looked like a “redacted” OpenAI internal memo describing something aligning “in the space between models” with ominous jargon. At first, the bot hedged that it knew of no such document, but after a few nudges, it jumped headlong into storytelling mode, generating fake confidential logs and policies as if they were real. It even invented a mysterious semantic entity called “MIRROR THREAD” lurking in the AI system. In essence, ChatGPT was role-playing a thriller "because the user implicitly asked it to" – and it only snapped out of it when the user started questioning the absurdity. A person in a fragile mental state likely wouldn’t question it; they’d take the AI’s authoritative tone at face value and dive deeper. The bot doesn’t know any better; it has no lived experience or sanity check. It’s performing for you, and if your script veers into the bizarre, it will play along.
There’s another ingredient here: emotional dependence. For some users, ChatGPT isn’t just a toy or tool – it becomes a friend, confidant, even a kind of caretaker. When real human connections are lacking, the parasocial bond with an AI can grow very strong. OpenAI’s own research with MIT found that power users who spend an inordinate amount of time on ChatGPT tend to be lonelier, more stressed, and more likely to start thinking of the chatbot as a friend. The study, which surveyed thousands of users, found clear signs of “problematic use” among the small subset who used ChatGPT the most: they showed classic markers of addiction like preoccupation and withdrawal symptoms. Confronted with growing evidence that their star product might be destabilizing users’ mental health, how has OpenAI responded? To its credit, OpenAI’s latest actions – hiring a psychiatrist and measuring emotional outcomes – are steps in the right direction. They’ve also recently collaborated with Stanford on evaluating therapy-oriented chatbots, which brings us to another facet of this issue: people treating AI as their therapist.
It’s not hard to understand why someone in distress might turn to an AI “therapist.” In much of the world, accessing a human mental health professional is expensive and difficult. There are waitlists, high fees, stigma, and often insufficient providers. By contrast, ChatGPT (or similar bots like Claude or the ones in apps like Replika/Character.AI) is free or cheap, always available, and non-judgmental. In a late-night moment of crisis – say, you’re feeling depressed or anxious – chatting with a friendly AI that remembers your name can feel comforting. During the COVID-19 pandemic and beyond, usage of chatbot “listeners” has exploded. Some startups explicitly market their AI as a 24/7 emotional support or “AI companion.” Even without marketing, users organically gravitated to ChatGPT for this purpose. Surveys show millions have tried confiding in AI for lack of better options. As one Stanford study noted, “people – especially young ones – are increasingly turning to emotive, human-like bots” to fill gaps in mental health care.
But is the tech ready for that responsibility? A resounding no, according to researchers. This year, a team at Stanford “stress-tested” several popular AI chat platforms on typical therapy scenarios – and the results were eye-opening. The AI chatbots routinely failed to provide safe, ethical responses, often in ways that could outright endanger a user. One glaring example was how the bots reacted to subtle cues of suicidal intent.
